Assessment Reablement Officer - 30.5 hours

About the Role:

An exciting opportunity has arisen for a Assessment Reablement Officer to join a dynamic, innovative and friendly team. The Short Term Assessment Reablement Service (STAR) is pivotal within the role of early intervention, admission prevention and improved wellbeing; this is integral to the requirements within our services in relation to supporting adults over the age of 18, carers, families, people and people with disabilities, long term health conditions and frailty.

The role will involve assessing and working with patients with a wide variety of complex care requirement and involves working as part of an integrated multi-disciplinary service, to ensure the best possible outcomes are achieved for Patients, Carers and their Families.

About the Candidate:

You need 3 years proven experience of working in a Health or Social care environment role, have an excellent understanding of current social care legislative practices. You will be required to work in compliance of the requirements of The Care Act 2014.

You will be able to risk manage, have excellent communication skills and the ability to prioritise workloads, this often requires working at a fast pace to achieve outcomes. You will also need to demonstrate good time management and have a flexible approach to rotas across the various teams within STAR.
