Team Manager – Transfer of Care Discharge Hub
We are seeking a motivated and solution-focused manager to lead our Transfer of Care Hub (ToCH) during a 6-month interim period.
This is an exciting opportunity for an internal candidate looking to further develop their leadership skills within a fast-paced, highly visible service at the heart of the health and social care system.
About the Role:
The Transfer of Care Hub plays a critical role in supporting timely, safe and person-centred hospital discharge, ensuring individuals receive the right support, in the right place, at the right time. Working across Adult Social Care, Health, Community Services and the Voluntary Sector, the service is central to improving outcomes for residents and maintaining effective flow across the system.
About the Candidate:
You will be a Qualified Social Worker and an experienced manager, with Substantial experience of managing complex caseloads, services, and safeguarding issues. You will have worked strategically across a range of health and social care services, with experience of multidisciplinary working in a range of settings.
